Nurse Practitioner Virtual Health

Major Responsibilities:

  • Evaluates, diagnoses, treats, and manages acute patient illness and diseases via on-demand virtual visits (i.e., E- and V-Visits).
  • Obtains medical histories, conducts visual examinations (as needed and as able) and assesses and determines differential diagnosis. Prescribes medication, therapy and treatments accordingly.
  • Interprets and evaluates patient data to determine management and treatment of patients. Consults with physician as appropriate. Orders, performs, and interprets diagnostic studies in the field of clinical specialty trained and determined competent by a physician.
  • Adjusts patient care plan by reviewing and monitoring treatments and therapy plans. Counsels, instructs and educates patients on physical and mental health including: diet, preventive health measures, health behaviors, self-care skills, disease, treatment options, and normal growth and development.
  • Provides care coordination, which includes managing all system-disqualified and clinically disqualified E-Visits via patient outreach (e.g., outbound phone calls, scheduling patient appointments). Appropriately communicates with virtual health team members during shift handoffs (e.g., relay status of potential patient follow-up calls, outstanding lab orders, unaddressed Epic In Basket messages, etc.). Covers lab results from orders created by virtual health providers. Handles all virtual-health patient follow-up calls.
  • Prescribes, recommends and regulates pharmacologic and non-pharmacologic therapies across multiple care settings, excluding controlled medications, that are warranted by patient s diagnosis or changing medical condition.
  • Prepares, dictates and signs patient care plans and summaries of patient care. Maintains records according to established standards and guidelines.
  • Applies clinical knowledge as requested by and for support of clinical contact center team members (e.g., assistance with calls from patients, pharmacies, facilities, etc.). Provides clinical direction to clinical contact center nursing staff regarding the continued care of patients. Follows through with nursing staff to ensure patient receives recommended care. Integrates with and must be available to contact center team members via chat, phone, or contact center workstation.
  • Actively participate in the virtual health quality reviews. Ability to work remotely from home and have access to a nearby Aurora facility/office in case of home technology issues. Maintain professional appearance and professional setting (home or work office) for all V-Visits. Work rotating holiday schedule. Flexibility to cover shifts when other virtual health NPs are on PTO or become unavailable.
  • Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient s status and interpret the appropriate information needed to identify each patient s requirements relative to his/her age-specific needs, and to provide the care needed as described in the department s policies and procedures. Age-specific information is developed further in the departmental job standards.

Licensure, Registration, and/or Certification Required:

  • Registered Nurse license issued by the state in which the team member practices, and
  • Advanced Practice Nurse Prescriber license issued by the state of Wisconsin, and
  • Basic Life Support (BLS) for Healthcare Providers┬ácertification issued by the American Heart Association (AHA) needs to be obtained within 6 months unless department leader has determined it is not required, or
  • Nurse Practitioner (AANP) certification issued by the American Academy of Nurse Practitioners (AANP), and
  • Nurse Practitioner (ANCC) certification issued by the American Nurses Credentialing Center (ANCC), and
  • Drug Enforcement Administration (DEA) registration issued by the Drug Enforcement Administration (DEA) Registration, and

Education Required:

  • Master s Degree in Nursing.

Experience Required:

  • Typically requires 2 years of experience in face-to-face clinic work with pediatric and adult patients.

Knowledge, Skills & Abilities Required:

  • Demonstrated high level of clinical proficiency, and excellent decision-making skills.
  • Demonstrated ability to work independently and as an effective member of a health care team.
  • Demonstrated ability to adapt to evolving technology and proficiency with the electronic medical record.
  • Excellent communication skills. Ability to effectively collaborate with and establish/build relationships with others.
  • Proven organizational skills and ability to prioritize effectively.

Physical Requirements and Working Conditions:

  • Must be able to stand, walk, bend, stoop, and twist continuously throughout the workday.
  • Must have functional speech, vision, touch, and hearing.
  • Must be able to:
  • lift up to 50 lbs from floor to waist.
  • lift up to 20 lbs over the head.
  • carry up to 40 lbs a reasonable distance.
  • Operates all equipment necessary to perform the job.
